Community Connections for Independent Living
The Community Connections for Independent Living (CCIL) program serves older adults living in Merrimack County, who may be at risk of losing their independence and having to move to a more restrictive care environment like assisted living or skilled nursing facility. Clients typically have limited to no support system and their changing or worsening health status requires connections to supportive services that will enable them to continue to live independently in a place of their choosing.
Our goal is to provide services to help older, isolated, adults remain living independently for as long as possible. The CCIL Social worker can meet with clients in their home to mutually develop a plan of action that meets their needs. The Social Worker coordinates resources and other financial assistance and maintains follow-up to ensure services are meeting their needs
