Homeless Street Outreach
The Homeless Street Outreach Team provides support and case management services to people experiencing homelessness or those who are at risk of becoming homeless. We engage with unsheltered individuals and families through partnerships with local service providers, 211 NH, the NH HMIS Coordinated Entry System, and regular unsheltered encampment outreach. We provide access to shelter & housing search information, food, and safety equipment such as tents, blankets, and basic survival items.

Other Requirements: Applicants must be experiencing homelessness, in accordance with State and Federal guidelines
Fast Facts
- Provides connections to resources based on a participant’s individual needs, using the housing 1st model.
- Anyone experiencing Homelessness is eligible, regardless of criminal background, substance use, or mental health.
- Our Street Outreach team connects directly with unsheltered participants through regular visits to known Homeless encampments.
- Ensures access to services for those who may not have transportation or access to technology.
