Coordinated Entry Program
The Coordinated Entry Program is designed to quickly rehouse homeless individuals and families. This program provides access to a variety of other community programs, with the goal of helping individuals and families become self-sufficient while minimizing the trauma caused by sudden dislocation and homelessness.

Other Requirements: Applicants must be experiencing homelessness, in accordance with State and Federal guidelines
Fast Facts
- Provides connections to permanent supportive housing resources based on a participant’s vulnerability and length of time experiencing homelessness.
- Anyone experiencing Homelessness is eligible, regardless of criminal background, substance use, or mental health.
- Our Street Outreach team connects directly with unsheltered participants through regular visits to known Homeless encampments,
- Ensures access to services for those who may not have transportation or access to technology.
